The top highest risk conditions for hospital readmission are heart failure, COPD, heart attack, hip or knee replacement, and coronary bypass. Other factors that lead to hospital re-admission within 30 days of discharge are four or more co-existing health conditions, being on four or more medications, being admitted to the hospital two or more times within the past six months, medication issues, and failure to attend follow-up appointments.
